Providence Health & Services Social Work and Behavioral Health Manager in Beaverton, Oregon
ElderPlace is a PACE (Program of All-inclusive Care for the Elderly) recognized by CMS as one of the most robust models of care for seniors with significant comorbidities. PACE is a program of all-inclusive services that helps keep older adults as healthy and independent as possible. We do this by providing comprehensive, high quality health and social services tailored to meet the unique needs of each individual.
ElderPlace serves patients age 55 or older certified by individual state to need nursing home care. ElderPlace and other PACE programs are able to provide the same level of care in the community by offering adult day center services, home healthcare visits, social services, and medical care that is provided by a medical team that includes MD/NP, RN, SW, RD, PharmD, PT/OT, SLP, CNA/CMA, PMHNP and drivers.
ElderPlace is the 5 th largest PACE site in the U.S. and has been providing care in OR for the last 28 years, serving over 1500 patients over 5 counties (Multnomah, Clackamas, Washington, Clatsop, Tillamook).
Providence is calling a Social Work and Behavioral Health Program Manager (1.0 FTE, Days) to Providence Office Park in Portland, OR.
In this position, you will:
Provide leadership and overall management of the PACE Social Work and Mental Health team operations.
Provide leadership in developing clinical and operations goals and objectives that support the PACE vision, and recommend long range plans for clinical and quality excellence.
Provide leadership at the local, regional, system and potentially national levels to advance Social Work practice, palliative, ethics and cross continuum collaboration, within and outside of Providence ministries, for the benefit of people we serve.
Provide consultation to Social Work Supervisors, Operations Managers, and Social Workers on complex patient, family, or financial needs
Lead quality initiative plans and meets within PACE and Providence wide
Involvement with community and regional committees
Possess budgetary acumen and provide financial oversight for PACE and ElderPlace
Required qualifications for this position include:
Master's degree in Social Work (MSW) from a CSWE Accredited School of Social Work
Licensed Clinical Social Worker (LCSW) license with the state of Oregon
Valid Driver’s License
10 years of work experience in PACE, medical social work and/or community-based care that includes:
overseeing social work practice, standards, regulatory guidelines and community engagement
providing professional social work services with the frail elderly that includes provisions of palliative care services
providing clinical supervision of social work staff for the purpose of licensure
managing budgets, monitoring expenditures, and determining resource allocations
Preferred qualifications for this position include:
Passion and experience with Geriatric care
Palliative and End of Life Care
Behavioral Health experience
